National Youth Event 2012 Trip Details

Over 220 UCC Youth from New England are heading to NYE!

        -Service trip itinerary   

        -Waiver form for work at the Cleveland Food Bank*
          This is only for those on the Southern Service Bus.  Turn this form in to your group leader and we will                collect them on the bus.

        -Self-Disclosure Questionnaire* 
          All adults who are going on the trip MUST fill this out and email it to their Conference Group Leader by             June 30.

NOTE:  Each group leader should bring a printed copy of the medical, covenant, and media release form for each member of his/her group.  In addition, please send one printed copy of the medical form to your State Conference Leader by June 30. See below for email addresses.
