Instructions For Completing The Congregational Self-Assessment Survey

• After downloading the Congregational Self-Assessment, please make enough copies for each member of your leadership team.
•  Have each member of your leadership team complete the form individually.

 1)If you are having a staff person or Coach work with your congregation individually, then send the completed self-assessment forms to Paul Nickerson at MACUCC 1 Badger Road, Framingham, MA. 01702. Nickersonp@macucc.org.

OR

 2) If you are joining a Cluster Group, bring you leadership team together and build a consensus around each answer and come to a composite score.  Also come to a consensus around the three areas of church life you wish to focus on.  Send the composite self-assessment to the same name and address as above.
